- K&K Claim Form / First Report of Accident (Online Submission): Completed by Trainer or Official
- K&K Fraud Statement & Authorization (Online Submission): Completed by injured party, parent or guardian
- K & K Claim Form / First Report of Accident (PDF form): To be submitted by trainer, official, coach
- U.S. Ski & Snowboard Fraud Statement and Acknowledgement (PDF form): To be submitted by injured party, parent or guardian
